Your First Visit

The purpose of your evaluation is to obtain a comprehensive understanding of your condition. We will ask you to fill out paperwork letting us know your current symptoms. If needed we have an on-site x-ray to help us make conclusive results on your exact problem.

Please Bring the Following With you for Your Visit:

     · List of current or recent medications and dosages

     · Name & address of your referring or primary care physician if
       you have one.

     · Name, address, and policy numbers of your insurance company.

Insurance

Please inform our appointment secretary of your health insurance coverage at the time you schedule your appointment. If your plan requires a referral or authorization, we ask that you obtain this prior to your scheduled visit.

Payment

You are responsible for payment of your office visit co-pay, coinsurance and/or deductible at the time of service. We accept cash, checks, visa, and mastercard.

Your health insurance plan may require that you receive care from a participating (in-network) physician. If we do not participate in your plan, your benefits will probably be reduced causing an increase in your payment responsibility.
